    Your First Appointment

    Please wear or bring comfortable, loose fitting clothing
    Please plan to be in our facility for 1 hour to 1.5 hours
    Please arrive 30 min before your initial scheduled appointment time in order to complete necessary paper work

    Additionally, please bring:

    • Your health insurance card
    • Physician’s order/prescription
    • Co-payment if applicable

    During your evaluation by the therapist, they will examine you and discuss your medical history, why you are seeking therapy care and any other concerns. Your therapist will complete a series of tests and measures as well as your ability to perform specific functional activities. The therapist will review their findings with you and together set a treatment plan designed to meet your goals.
